Housing Case Manager - Mt. Vernon, NY

JOB TITLE

SUMMARY
: Provide housing case management services to homeless families residing in a Tier II family shelter to rapidly secure permanent housing.

DUTIES AND RESPONSIBILITIES
:

  • Meet with every family within one week of intake to identify housing needs and preferences.
  • Maintains accurate, complete and current files on all clients on caseload.
  • Responsible for implementing home finding plans, assessments and progress notes.
  • Maintains contact with all necessary collaterals and attends appropriate meetings.
  • Help families register their school-aged children for school and monitor attendance.
  • Provides crisis intervention services as needed.
  • Conducts bi-weekly unit inspections.
  • Participates in monthly Town Hall meetings.
  • Maintain demographic and other needed information regarding all clients served for use in statistical reporting through our ClientTrack Homeless Information System.
  • Provides skill-building activities to enhance clients ability to obtain and retain permanent housing.
  • Help participants complete and file housing applications, complete and file applications for housing subsidies, secure needed documents, and schedule appointments to see units;
  • Help participants secure moving costs from DSS, including broker s fees, security deposits, first month s rent, and furniture allowances; and
  • Help residents arrange utilities and utility payments.
  • Encourage participants to comply with the Employment Plans and Independent Living Plans that will be developed by DSS.
  • Other duties as assigned.

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS
:

Bachelor degree required. Two years of related and/or applicable experience working with children and families. Fluency in Spanish is highly desirable. Ability to work some evenings and occasional weekends. Excellent and demonstrated writing ability with strong spelling, grammatical and punctuation skills, and strong oral communication skills. Advanced knowledge of MS Word, Excel, PowerPoint and database management skills.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
